'Welcome Home with Walnut' aims to offer you a trouble free return home from hospital or respite care. We can meet you at home or accompany you on your journey. We can check your home is ready for your return and shop for the basics before you arrive.
Walnut Care can help with personal care if needed and we will help you to regain as much independence as possible, with a tailored made re-ablement programme.
